ChipotleFeedback Survey – Claiming The Prize
The Sponsor or its authorized designee will notify each potential winner by e-mail at the e-mail address provided at the time of entry. A form Affidavit of Eligibility/Liability & Publicity Release, as well as a tax acknowledgment, will need to be completed by all potential winners before they are declared the winner.
Before being declared a ChipotleFeedback survey winner, a potential Canadian resident will have to correctly answer timed mathematical skill-testing questions, which will be provided with the email notification, or the Prize will be forfeited. If you are a Canadian resident, a Declaration of Eligibility, Liability, and Publicity Release is required.
The potential winners must complete and return a signed and notarized affidavit/declaration within seven calendar days from the date they are sent the affidavit/declaration, or they may forfeit the Prize.
The Sponsor will use its best efforts to notify all potential winners, but the Sponsor is not responsible for any undelivered emails, including, without limitation, any Promotion e-mail, including any notification e-mail to the winner, that may be diverted to a spam or junk folder.
Upon verification of the winner’s eligibility, the sponsor will attempt to mail the Prizes within 6 to 8 weeks. The Promotion Entities will not be liable for any delays in awarding a Prize. The winners are solely responsible for any federal, state, provincial, territorial, and local taxes imposed by the government on the acceptance of the prize. The sponsor retains the right to terminate, cancel, modify, or suspend the promotion at any time without prior notice.
